We are thinking of moving to spain but have concerns about getting started on the health systems. Everybody we talk to say how wonderful it is, but nobody will tell us just how we do it and nobody offers any advice of any help that is available .

I find your comment hard to believe there is loads of advice available from just a simple search of the internet, secondly Spain Expat Forums have many people willing to share their experiences good and bad at both applying, transferring and living within the Spanish Social system,
If you are planning on moving to Spain you need to do some quality and proper research, and if you fail at this simple hurdle, it is not the place for you!!
