The Skin I Live In, starring Antonio Banderas

Ever since his wife was burned in a car crash, an eminent plastic surgeon (Banderas), has been experimenting to creating a new skin with which he could have saved her.

In addition to 12 years of work, he needed three things: no scruples, an accomplice and a human guinea pig.

After a confusing start the film gathers pace and ends with a great twist. It gets more normal (ish) as the story progresses.
